"Teen Wolf" Dylan O'Brien factored in an accident on March 17 while filming the upcoming installment of the "Maze Runner" movie series, titled "Maze Runner: The Death Cure."
The actor suffered "concussion; facial fracture and lacerations," according to WorkSafeBC.
The report did not name O'Brien, but the accident the actor suffered fits the description previously released regarding the incident.
"A worker was leaving the filming area on a motorcycle after completing a stunt scene," the report reads. "The motorcycle went into a slide while turning onto a side road. The worker struck his head on the pavement and temporarily lost consciousness."
"Maze Runner" director Wes Ball confirmed the incident a couple of days after news about it surfaced online. He posted a photo of a handwritten note, which said, "Well, it's been a whirlwind of emotions these past few days. I've been overwhelmed with feelings of anger and sadness and guilt."

Ball continued: "But ultimately, I find myself left with just a deep love and respect for Dylan. He is one tough cookie. I'm sorry for the grief this accident has caused his friends and family as well as my wonderful cast and crew."
Ball added: "It's scary seeing your friend get hurt, but fortunately, Dylan is going to be just fine. After a few weeks of rest and recovery, he'll be back up and running to finish our adventure together. All the best."
O'Brien has yet to release a statement about what happened.
20th Century Fox, on the other hand, had this to say about the incident: "Dylan O'Brien was injured yesterday while filming 'Maze Runner: The Death Cure' in Vancouver, Canada. He was immediately transferred to a local hospital for observation and treatment. Production on the film will be shut down while he recovers. Our thoughts go out to Dylan for a full and speedy recovery."
"Maze Runner: The Death Cure" is set to hit theaters Feb. 17, 2017.
